Guest Facing Bilingual Case Manager for PADS of Elgin
Position Summary
This team-oriented, exceptionally organized, coachable, and empathetic individual is responsible for providing guest-facing case management to PADS of Elgin guests for shelter programs. Case Managers also ensure guest compliance with stated program goals and assist guests with achieving self-sufficiency and ultimately obtaining sustainable housing. They will also share community resources to increase awareness and access to programs for the guests. Additionally, this person works very closely with all staff and collaborates with community partners for wrap-around care. This position requires in-person contact with guests.
Duties and Responsibilities
- Provide comprehensive case management services and shelter to individuals and families experiencing homelessness.
- Meet with guests weekly to share progress on goals and any areas of need.
- Administer the MAP Assessment to each guest and determine placement on housing prioritization list.
- Assess new guests so that PADS of Elgin can meet their needs effectively.
- Conduct a background check for new guests.
- Schedule and coordinate the intake process for new guests.
- Establish and track individualized goals for each guest.
- Serve as a resource for educational, vocational, and medical needs.
- Electronically document case notes of all guest interactions promptly using both internal communication systems and the Homelessness Management Information System (HMIS).
- Advocate on behalf of guests as they search for housing.
- Provide case management services for Rapid Re-housing guests for the duration of their program.
- Make referrals as needed to other local social services for guest needs.
- Attend meetings with other local service providers.
- Assist guests with scheduling appointments and coordinating transportation, etc.
- Demonstrate alignment with our mission statement to provide compassionate care and support for individuals experiencing homelessness.
- Support Shelter Care Coordinator Team with enforcement of shelter rules and client expectations.
- Support Front Office Team with outreach services.
- Support the Development Team annually at one of the two major fundraising events, as well as one other event.
- Other duties as assigned.
Job Requirements
- Team-oriented, exceptionally organized, coachable, and empathetic individual who can work with a variety of departments, personalities, and priorities.
- 2 years of experience in a human services capacity or related experience preferred.
- Associate’s or bachelor’s degree in a human services field (psychology, social work or a related field) preferred
- Proficiency in English and Spanish (oral/written).
- Ability to work independently and collaboratively while prioritizing multiple projects and deadlines simultaneously.
- Ability to establish and maintain professional boundaries in working with guests.
- Ability to work with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental illness.
- Ability to effectively use conflict resolution and de-escalation techniques.
- Ability to apply rules fairly and consistently.
- Working understanding of standard housing programs (Rapid Re-housing and Shelter Diversion) preferred.
- Demonstrate alignment with our mission statement to provide compassionate care and support for individuals experiencing homelessness.
- Excellent interpersonal and intercultural communication skills.
- Proficiency in relevant software tools to efficiently document client case notes and interactions.
- Excellent oral communication skills with the ability to participate in public speaking events.
- Excellent written communication, research, and organizational skills.
- Must be proficient with Microsoft Outlook, Word, and Excel.
- Ability to speak knowledgeably and enthusiastically about PADS, its mission, programs, and initiatives.
- Able to use good judgment and identify changing program needs.
- Must be available to work some weekends and evenings as required.
- Valid driver's license and vehicle insurance required.
- Must be willing and able to drive PADS of Elgin van for case management needs.
