Open Door Health Center is a regional, patient centered health care organization that provides services including education, prevention, diagnosis, and treatment services to those who have or are at risk for Sexually transmitted Infections including HIV/AIDS, in addition to the greater LGBTQ community. Open Door provides medical care, mental health care & support services to people living with HIV/AIDS and their families. Our centers located in both Elgin and Aurora addressing primary care and specialized services to eliminate disparities in healthcare.
Job Description
Medical Case Management is the provision of a range of client-centered services focused on engaging HIV+ clients into medical care and medication adherence with a focus on viral suppression to improve or maintain a quality of life. Medical Case Management includes maintaining data compliance with Standard Operating Procedures (SOPS) of the Case Management funders; to assess eligibility and assess needs through the creation of a client-centered plan of care every six months; The HFS-DRS Home Service Program (HSP) Case Manager is responsible for assessing customer referrals and managing the home service program through HFS. This program is designed to allow customers who qualify for services to remain in their homes and not be admitted to a Nursing facility. The DRS HSP Case Manager may also monitor a portion of the DRS HSP customers who qualified for Managed Care through the State of Illinois.
- Provide ongoing Medical/DRS Case Management and support services to eligible clients each month per funding source.
- Conduct initial Eligibility Assessments and Re-eligibility Assessments at a minimum of every 6 months or annually as dictated by funding source.
- Development of a comprehensive, individualized Care Plan jointly with the client including hours of service, short and long term goals, with measurable tasks focused on attaining, maintaining, and achieving positive identified health outcomes.
- Continuous client monitoring to assess efficacy of Care Plan.
- Monitor and refer needed resources for care; individual providers and home makers as needed.
- Initiate and/or refer client for benefit’s counseling, assisting eligible clients in obtaining access to public and private programs for health insurance coverage (Medicaid, Medicare, COBRA, Medication and Premium Assistance programs, ACA Insurance Marketplace/Exchanges).
- Provide ongoing assessment of client adherence providing treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments.
- Work in conjunction with medical care providers (e.g. case conferencing, care conference) ensuring comprehensive medical care is provided and monitored based on client individual medical and psycho social needs.
- Refer and link clients to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Monitor client follow through with identified services.
- Demonstrate cultural competency when working with clients who may be HIV+, LGBT, homeless, indigent, substance users, mentally ill, non-English speaking, or otherwise marginalized and/or underserved.
- Ensure the protection of and security of a consumer’s personal, confidential and identifiable information in a professional and responsible manner, according to the standards and requirements of the Health Insurance Portability and Accountability Act (HIPAA).
Qualifications
- Bachelors’ Degree in Social Services, Psychology, or other social service or healthcare related field
- Minimum 2 years’ experience in case management or social services
- Multitasking, organization and time management skills.
- Ability to handle confidential matters with utmost integrity.
- Working knowledge of computers and common software programs.
- Proficient in Microsoft Suites; Outlook, Word, PowerPoint and Excel.
