PADS of Elgin is committed to providing safe shelter, support services, and compassionate advocacy to the individuals and families experiencing homelessness in Dundee, Elgin, and Hanover Townships. Our goal is to break the cycle of homelessness by addressing the comprehensive needs of the local homeless population. PADS provides guests with a safe place to sleep, showers, laundry facilities, nutritious meals/snacks, and case management to help guide them towards a path to self-sufficiency. ESSENTIAL DUTIES & RESPONSIBILITIES

PADS of Elgin is seeking an Outreach Case Manager - flexible schedule Monday through Saturday 8am to 8pm.

This position is responsible for providing high-quality services for individuals experiencing homelessness.

This position requires street outreach, case management, psychosocial rehabilitation services and crisis intervention at PADS' location and in the community. This requires the ability to build trusting relationships with clients and community organizations.

Assess the needs of service and provide resources and recommendations for treatment.

Assist clients with housing, medical and psychiatric help.

Advocate for clients on all levels.

Complete all documentation. Knowledge of HMIS system. Attend weekly/monthly meetings.

Engage in all of PADS' trainings, consultations and supervision.

Implement appropriate action plans so that guests can become independent and find employment and permanent housing.

Help build life skills with clients.

Conduct weekly/monthly follow-up meetings to ensure compliance with action plans.

Make referrals as needed to other local social services organizations for assistance with client needs.

Attend meetings with other local service providers.

Assist clients with daily needs such as transportation assistance, doctors appointments, etc.

SKILLS NEEDED

Demonstrate compassion, understanding and commitment to the organization’s mission, vision, and core values.

Ability to be highly self motivated.

Problem solving skills. Ability to work effectively in constantly changing and sometimes demanding environment using conflict resolution and de-escalation techniques.

Ability to assess emergency situations and respond effectively.

Ability to establish and maintain professional boundaries in working with guests.

Ability to work with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental illness.

Ability to communicate well both orally and in written form and follow detailed instructions.

Ability to accurately record information for required guest and staff documentation.

Bachelor's Degree in social work or similar field required. Master's Degree preferred.

Mental Health Standard of Rule 132.

Previous experience working with individuals experiencing homelessness, substance abuse and mental health challenges. Life experience welcome.

Valid driver's license and insurance required.

A bilingual candidate is preferred. Must be 21 years of age.

Job Type: Full-time

Pay: $19/hour; laptop and phone provided. Mileage reimbursed.
