VNA Health Care received recognition from the US Department of Health and Human Services Health Resources and Services Administration (HRSA) for its achievements in providing high quality, comprehensive patient health care. VNA will receive $171,372 in funds that are designated by HRSA to expand current quality improvement systems and infrastructure and improve primary care service delivery. The funding community health centers receive is directly related to the level of quality demonstrated.

“VNA’s funding was awarded for demonstrated improvements in one or more clinical measures, having high clinical performance compared to other health centers, and for maintaining primary care medical home status.  I think our patients will be pleased to know that once again, VNA was named a Health Center Quality Leader,” said Claire Dobbins, Vice President of Clinical Services & Chief Clinical Officer at VNA Health Care.

“This award highlights our dedication to helping people reach their health goals,” said Linnea Windel, President/CEO of VNA Health Care. “Our approach to healthcare focuses on several key areas, including enhancing the primary care provider relationship, thoughtful care coordination, and frequent health promotion initiatives. We couple these patient-centered efforts with a systematic approach to quality and the right technology to support care and communicate with our patients.”

VNA Health Care operates 10 clinics in suburban Chicago and serves over 65,000 people a year. VNA is nationally ranked as a top performing health center as a result of patient outcomes such as prenatal care, adolescent weight screening and counseling, vascular disease treatment, blood pressure control, healthy birth weights, and asthma care. VNA’s combination of convenience (by appointment or walk-in), patient-centered technology and expert care delivered with respect and compassion have led to these results. VNA is accredited by The Joint Commission.
